At ChangeInc we recognize that elder care needs are unique to each individual and family. That is why we offer a wide variety of solutions to manage your home care service. We are committed to doing everything we can to ensure your loved one is comfortable, secure, and independent at home with the support of our Elder Care Managers and Direct Care personnel.
ChangeInc is a credentialed provider under the Connecticut Home Care Program for Elders (CHCPE) operated by the Department of Social Services. We currently offer the following services:
- Personal Care Assistance
- Live-In Personal Care Assistance
- Bill Payer
- Mental Health Counseling
Personal Care Assistance
This service enables qualified individuals in the health care industry to provide hands on assistance with activities of daily living including personal hygiene care, cleaning, and meal preparation. We provide daily or live-in care.
The role of companion is to deliver non-medical care to elders in accordance with their therapeutic goals established in their care plan. Often our companions provide transportation to medical appointments, grocery shopping, and to various recreational/leisure activities to aid in socialization.
Homemaking duties consist of general household activities including but not limited to: laundry, dishwashing, vacuuming, dusting, meal preparation, and general cleaning.
This service includes heavy household work to ensure the safety and cleanliness of an individual’s home. Tasks might include: yardwork, deep cleaning, and moving furniture for safety.
Bill Payer’s fulfill the financial management needs of our elder care individuals. Qualified ChangeInc bill payers aid elders at home with budgeting, organizing, and paying household bills as needed.
Mental Health Counseling
ChangeInc has LCSW, MSW, LMFT, and LPC's that work with individuals, families, and groups to address and treat emotional and mental disorders and to promote mental health. They are trained in a variety of therapeutic techniques used to address issues, including depression, addiction and substance abuse, suicidal impulses, stress, problems with self-esteem, and grief.
